Thank you for choosing Capital Family Physicians. To schedule a physiscal,  please call our office during normal business hours. There are a limited number of physical appointments available daily, so please understand that you may have to schedule several weeks in advance to secure your preferred time frame.

Please note:

• YOU MUST BE FASTING FOR YOUR PHYSICAL – WATER ONLY FOR 8 HOURS PRIOR TO YOUR APPOINTMENT
• If you do not show up for your physical or cancel your physical with less than 24 hours notice you will be charged a $75 no show/late cancellation fee
• The following forms must be filled out every year for your physical:
  • Physical Form – Patients with commercial insurance (NOT MEDICARE)
  • Welcome to Medicare Visit Form – Patients that are in the first 12 months of Medicare Part B coverage
  • Medicare Annual Wellness Visit Form – Patients with Standard Medicare (Red/White/Blue Card) Any Wellness Visit AFTER your Welcome to Medicare Visit
  • Medicare Advantage Physical Form (Your coverage is through BCBS Blue Medicare, UHC Medicare Advantage, Humana Medicare Advantage, Coventry/Advantra Medicare Advantage, etc…)
