Insurance

Capital Family Physicians accepts most major insurance plans including but not limited to the following:

  • Aetna (Most Plans – Excluding Duke Select and Duke Basic)
  • Blue Cross Blue Shield (All Plans)
  • Cigna (Excluding Cigna Connect and Cigna SureFit)
  • MedCost Preferred
  • United Healthcare
  • Wellpath

We ask that all patients present their insurance card at each visit. New patients must present insurance cards at their initial visit. If you do not have an insurance card or some proof of coverage, you will be responsible for full payment of the charges for that visit. You can then file your claim for reimbursement. If your insurance requires that you sign up with a “Primary Care Physician” (PCP) this must be done prior to being seen by the provider.

If your insurance requires a co-payment, you must make that co-payment at each doctor visit. We are under contract with your insurance carrier and are required to collect this at the time of your visit.

If you have a deductible, you are required to pay at the time of service until your deductible is met.

If you have commercial or private insurance which we do not accept, or no insurance, you are responsible for payment in full at the time of service. You will be given a receipt to file for reimbursement.

We file all Medicare claims.  We file secondary insurance claims for Medicare patients only.

It is important that you inform us all of insurance changes. This allows us to file claims in a timely manner and prevents billing errors.
