Capital Family Physicians accepts most major insurance plans including but not limited to the following:

We ask that all patients present their insurance card at each visit. New patients must present insurance cards at their initial visit. If you do not have an insurance card or some proof of coverage, you will be responsible for full payment of the charges for that visit. You can then file your claim for reimbursement. If your insurance requires that you sign up with a "Primary Care Physician" (PCP) this must be done prior to being seen by the provider.

If your insurance requires co-pay, you must make that co-payment at each doctor visit. We are under contract with your insurance carrier and it is required that we collect this at the time of your visit.

If you have a deductible, you are required to pay at the time of service until it is met.

If you have commercial, private, or no insurance, payment in full for each visit is due at the time of your appointment. You will be given a receipt to file for reimbursement.

We file all Medicare claims as well as insurance secondary to Medicare.

It is important that you inform us all of insurance changes. This allows us to file claims in a timely manner and prevents billing errors.
