Patient Responsibilities
  • Patients should arrive 15 minutes prior to their scheduled appointment. Late arrivals (in excess of 15 minutes past appointment time) may be asked to reschedule the missed appointment.
  • Please present your insurance card at each visit.
  • Please bring an updated list of ALL medications you are taking OR bottles for these medications with you to each appointment.
  • Questions regarding your benefits should be directed to your insurance carrier or your employer's human resources department.
  • Scheduled appointments are preferred. Walk-in patients will be evaluated and worked into the schedule if an appointment is necessary and a physician is available.
  • New allergy patients must be seen by one of our providers prior to the first allergy injection. All allergy patients must bring an order from your allergist, instructions, and serum before any allergy injections can be administered.
  • All patients under 18 years of age must be accompanied by a parent or legal guardian or have either signed parental/legal guardian consent or verbal authorization from a parent or legal guardian for treatment.

Hospital Admissions
If you need to be admitted to a hospital, a hospitalist physician will be in charge of your care. A hospitalist is a physician who practices medicine solely at a hospital. The practice is informed of the treatment you receive and notified upon your discharge.

A physical form can be downloaded from the Forms section of this website and should be completed prior to your appointment. If you are unable to download the form, please arrive 15 minutes prior to your appointment in order to complete the form.

Nurse Visits
Nurse visits are for injections, blood pressure checks or other procedures predetermined by your provider. Appointments of this nature may be made the same as any other physician visit.

Phone Calls and Voice Mail Messages
All phone calls and voice mail messages will be attempted to be returned within 24 hours. If you have an urgent medical need, please call our main line and explain the situation to the operator so that he/she can direct the call. If you find yourself in a true medical emergency, please call 911 or proceed to the nearest Emergency Department.

Monday - Thursday 8am - 3pm
Friday 8am - 1:30pm
